PURPOSE

To provide high quality, supportive care services to IHHC patients and their families and to ensure that the changing needs and goals of the patient are being met.

POLICY

The Island Home Health Care believes that the right to a person centered approach and continuity of care is a key principle in modern health care and must be respected at all times.

The right and preferences of the persons served are in the center of all decisions, and all changes are discussed with and communicated to the persons served, family; support system as well as communicated to referral sources and other stakeholders as applicable.

PROCEDURE

1. All changes are discussed with the care team, persons served, family; support system as applicable.

2. In all decisions IHHC, keep the care centered on the needs and wishes of the client served, the family and support system.

3. Any changes in service needed, available choice and decisions affecting the care, e.g. change of personnel, changes of service intensity, continuation or discontinuation, are discussed and shared with the persons served, family.
