1. To follow the rules and regulations of Island Home Health Care.
2. To show respect and to Island Home Health Care staff without discrimination to color, religion, nationality or ethnicity.
3. Should give accurate information about personal details and past medical history as well as to inform the IHHC staff of any treatments and medications that they are taking and history of allergy or of allergic reaction to any medication.
4. Be accountable for their own actions if they decide not to follow the health care provider instructions and/or treatment plan and recommendations.
5. Permits Island Home Health Care to renew in your behalf to the insurance company or file appeals in case of rejection and cooperate to the fullest extent possible.
6. Notify Island Home Health Care if you will not be unavailable for a scheduled visit in advance so that the plan of care will be revise in accordance to your availability.
7. Make a family member or a significant other available who will assume a primary caregiver role when Island Home Health Care staff is not in your home.
8. Inform the medical staff if they have any special requirements for discharge and are requested to be prepared to go home once medically discharged from the hospital.
9. Is accountable for the payment of any deductible or medical services provided by Island Home Health Care excluded from the insurance scheme of the third party payer.
