The Island home healthcare team is working together on each person serve medication plan, in conjunction with the rest of their treatment, which is vital to their recovery. Medication management is part of every person serve’s plan of care. On our initial visit a clinician will complete comprehensive medication reconciliation.
Medication education is provided to every patient based on each medication the patient is prescribed. This includes its purpose, how and when to take it and how much of the medication to take.
Medication reconciliation ensures that we account for every possible side effect and interaction that our patients’ medications could possibly cause. We instruct the patient or patient’s families on symptoms that need to be reported to the physician based on the medication side effects and possible interactions with other medications or food. Our team assists the physician or arranged referrals in the management of the anticoagulation therapy by obtaining lab works and providing education on how to manage diet, activity level and improve home safety.
