EMPLOYEMNT CONTRACTS


1. PURPOSE:

 .1 The purpose of the policy is to prescribe the terms and conditions under which
   contracts of employment should be administered, not intended to be used as an
   alternative to the recruitment and selection procedures.

2. POLICY:

 2.1 Island Home Health Care recognizes that its most valuable asset is its Human
   Resources. A great deal of time and money is invested in the recruitment, training and
    development of employees and as such, every effort should be made to retain those
   employees who

3. DEFINITION:

 3.1 Contract of Employment- a written contract entered into between Island Home Health
   Care and the employee, employed for the purpose of performing certain specified work
   for a certain specified period and whose complete terms and conditions of employment
   are contained in written contract.

4. PROCEDURE:

 4.1 Employment Procedures:
    4.1.1 During the course of the recruitment process, at such time as an individual Has
   been selected as an employee of Island Home Health Care, the standard employment
   contract will be prepared for the individual.
   4.1.2 As an integral part to the to the employment contract, a Master Schedule will be
   prepared in which all data pertinent and unique to that particular employee will be , set
   forth. Making the appropriate entries on the Master Schedule will indicate the
   contractual benefits allowed to that employee.
   4.1.3 The employee and an authorized representative of Island Home Health Care will
   sign the Master Schedule and each page of the contract, thereby constituting
   completion of the agreement.

 4.2 Changes in Salary or Position
   4.2.1 At such time as the employee become eligible and is granted merit increases,
   promotions, alignment adjustments, etc. a contract Renewal Form (CRF) will be
   prepared by the Human Resources reflecting all data pertinent to the change.
    4.2.2 The Human Resources will process the PAC Form and route it for approval.

 4.3 Contract Status Change
   4.3.1 At such time as it is necessary to effect a change in the data of the contract, the
   nature of the contract, or the benefits afforded to the employee under the Human
   Resources specifying the requested contract change will initiate a CRF Form.
    4.3.2 The CNF form will be routed through normal channels for consideration and
   approval.

 4.4 Termination:
   4.4.1 At such time as it is determined that the Employee will be separating from
   employment with the Island Home Health Care, a CNF form will be initiated by the
   Human Resources.
   4.4.2 The CRF form will be processed through the Human Resources Department for
   approval.
    4.4.3 Subsequent to approval of the CRF, the agreement between the employee and
   the Island Home Health Care will be considered cancelled as of the effective date of the
   separation.

5. RESPONSIBILITY:

  5.1 Senior Nurse In Charge / Human Resources:
    5.1.1 Evaluate the abilities and performance of the new staff.
    5.1.2 Evaluations should be done no later than 5th month from the date of hire.
    5.1.3 Evaluations may be done in writing and sent to HR so they may be placed on the
   employees personnel file.
    5.1.4 The employee should receive a copy of any written evaluations.
